Website privacy policy
Last updated October 20, 2026
This policy explains what information this website collects, why, who receives it, and the choices you have. This website is operated by Lens Society. The legal entity is named in the disclosure at the bottom of every page.
This policy covers the website only. It does not describe how we handle your medical records once you are a patient. That is covered by our Notice of Privacy Practices (see “Patient records” below).
We wrote this in plain language. It describes how the website works today, and we will update it when that changes. It is not a promise that every law in every place applies in the same way to you.
What the website collects
When you send a request
The request form on our contact page asks for:
- your name
- how you want us to reach you (call, text or email)
- your phone number or email address, depending on that choice
- preferred days and times, if you give them
- what the request is about: an appointment, eyewear, or an insurance question
- optionally, a frame brand, style or model you are interested in
- optionally, the name of your insurance carrier (the company name only)
The form does not ask for health information, insurance member ID numbers, dates of birth, or photos of cards, and please do not include them. If we need those details, we collect them by phone, in person, or through the practice’s patient system.
Even without health details, a request can show that you may be looking for eye care, so we treat every request as confidential.
When you browse
- Visit counts. We use Cloudflare Web Analytics to count page views and see which pages are useful. It does not use cookies or similar storage in your browser, and it is not used to identify you or follow you across other websites.
- Security and delivery. Like any website, our hosting and security provider processes technical information such as your network (IP) address and browser type so pages load and the site stays protected from attacks.
- Bot check. The request form uses Cloudflare Turnstile to tell people from automated spam. To do that, Cloudflare processes information about your browser and device, such as your IP address and browser signals. Cloudflare’s own privacy policy describes that processing.
- Request limits. To stop abuse, the website may briefly keep a count of how many requests come from a network address. That count is not linked to your name or request.
Why we use it
We use request information to:
- contact you the way you asked
- schedule or confirm an appointment, answer an eyewear question, or look into an insurance question
- keep the form working and free of spam
We use visit counts to improve the website. We do not use any of this information for advertising.
Who receives it
- Our office staff. Requests are delivered to the office by a secured email service that is set up for health care use. Only staff who handle requests read them.
- Service providers. Our website host, security provider and email service process information only to run those services for us.
- When the law requires it. We may share information if the law requires it or to protect someone’s safety.
We do not sell your information. We do not share it with others for their marketing. The website has no advertising pixels, social media trackers or ad networks.
Texts and email
We do not send marketing texts from the website. If you choose text as your contact method, we will text you only to respond to your request, for example to confirm a time. Message and data rates from your mobile carrier may apply. If you would rather not get texts, tell us and we will use another method.
If you choose email, our replies are short and avoid health details. Regular email and text messages are not fully secure, so please keep health details out of them and tell us by phone or in person instead.
How long we keep it
We keep request information for no longer than needed to respond and schedule, and we review this inbox regularly. Once a request has been answered or entered into our scheduling system, we remove it from the inbox. If you become a patient, the information that goes into your patient record is kept as described in our Notice of Privacy Practices and as the law requires.
Patient records
If you become a patient, your medical and billing records are protected under HIPAA. Our Notice of Privacy Practices explains how we use and share that information and your rights, including your right to see and get a copy of your records. The notice is available at the office, and you can ask us for a copy at any time.
This website policy and the Notice of Privacy Practices are separate documents. If they ever seem to conflict about your patient records, the Notice of Privacy Practices applies.
Other websites we link to
Some links take you to services run by other companies, such as an online appointment scheduler, an online frame store, maps, or social media. Their own privacy policies apply to anything you do on their sites. Read those before you enter information there.
Security
We use reasonable safeguards, including encrypted connections to this website and a secured delivery service for requests. No website or email system can be made perfectly secure, so please do not send health details, ID numbers or card photos through the website.
Your choices
You can ask us what request information we have about you, ask us to correct it, or ask us to delete a request. We will honor reasonable requests unless we need to keep the information as part of a patient record or because the law requires it. To ask, use our contact page.
Children
This website is not directed to children under 13, and we do not knowingly collect information from them through the website. A parent or guardian should send any request for a child. If you believe a child has sent us information, contact us and we will delete it.
Changes to this policy
We will update this policy when the website or our practices change. The date at the top of this page shows when it last changed.

If you have questions about this policy, reach us through our contact page or stop by the office. Directions are on our visit page.
